2010 Faith UMC VBS Registration Form


PLEASE FILL OUT ONE FORM PER CHILD AND SUBMIT ELECTRONICALLY.  THE REGISTRATION FEE MUST BE LEFT AT THE CHURCH  OFFICE BY THURSDAY, JULY 15, 2010.

The registration fees for the 2010 VBS are:

  • $8 per child OR $16 per family on or before July 15th
  • $12 per child OR $24 per family after July 15th

If you would like a CD of the High Seas music, the cost is $10.

Friends and guests of members are not considered family. They must register and  pay the registration fees separately.  Children must be 4 years old by start of program.

By supplying your name, you give permission for the named child to participate in Faith United Methodist Church vacation bible school.
